Observed a hand sink in the ware washing area to be disconnected. Per the PIC the mop sink is clogged where the hand sink drains to so it is not connected.

Per the PIC there is a work order in. Ensure this is fixed by next routine inspection.

Observed a hole in the wall as you walk in the kitchen. Observed multiple ceiling tiles throughout the facility with water spots.

Ensure the hole is patched by next routine inspection to prevent pest harborage.

- 6-501.11: PHYSICAL FACILITIES shall be maintained in good repair. - V
Inspection Comments -Make sure doors are shut after taking trash out.

-Ensure dumpster is closed when items are taken out to prevent nuisance pests from getting in the dumpster and surrounding area.

-Keep up with cleaning of the ice machine so baffle does not get soiled. Baffle was not overly soiled at time of inspection but there was minor residue present.

-Staff was very friendly and accommodating. Thank you.
